Please request a consultation where you will be sent a medical form prior to the consultation being accepted. Once the medical form has been completed and returned we will assess if the treatment is suitable for you based on medical grounds and confirm the consultation date that you have booked. After a consultation has taken place and you decide to continue with the treatment we will book you in for your treatment date.
